Lavaca School Based Health Center

Lavaca Wellness Center was established through a School-Based Health Center grant awarded by the

 Arkansas Department of Education – Office of Coordinated School Health.

 

Parents/Guardians: If your student needs to be seen at the Wellness Clinic, even if they are home sick, please contact your child’s school nurse during school hours to make the appointment. School nurses can make same-day appointments. If you schedule an appointment directly through the clinic you will not be guaranteed a same-day visit.

Your child should be registered with the clinic prior to an appointment being made, and a current year Parent Consent Form must be on file with the school in order for school personnel to transport your child to and from an appointment upon your request. Registration and Consent Forms are updated annually.

If you would like to register your student with Wellness Center providers, you may print the registration packet at the link below, fill out and return to your child’s school. You may also obtain a copy of the registration packet at any of the school offices.
